How do I add a form to my page?

Create the form. See creating forms.
Click "Page Content" from the left column.
Click the page the form should be displayed on.
Click the "Form" tab located at the top of the page between "SEO" and "Coupons."
Click the "Get code for page" link next to the form you desire.
After the "Get code for page" link is clicked, copy the text in the pink box at right by right click or control and C.
Click the "Content" tab located first before "Forms".
Paste the code into the text area where you want the form to display.
Click the "Save Changes" button.
